FINDINGS AND IMPORTANT DOCUMENTS


Dear couple,

In order to ensure your quick diagnosis, we recommend bringing the following with you to the initial consultation:

A) Previous findings for the woman (not older than 6 months)

  • Blood count and CRP
  • Hormone status (taken between 1st and 5th cycle day): AMH, FSH, LH, E2, Prol, Prog, Testo, TSH
  • Coagulation: PTT, PTZ and APC resistance
  • Rubella titre, blood group and rhesus factor
  • Cancer smear (PAP): findings from the gynaecologist, not older than a year

B) Previous findings for the man (not older than one year)

  • Spermiogram

C) Additional findings for both – if available:

  • Genetic investigations
  • Findings from other specialists
  • Operation reports
  • Reports on preliminary tests in other institutes
  • Immunological investigations

If you would like to have treatment in our Kinderwunsch Institut Dr Loimer, the following documents must be submitted:

  • ID of both partners (Driver’s license, identity card or passport)
  • Proof of address from at least one of the partners (only for fund patients) not older than one month
  • Copy of the marriage certificate or
  • A notarial act for unmarried couples

Cost of the notarial act: EUR 265, for couples at the public notary’s office Dr. Gottfried Stieber, Spittelwiese 8 in 4020 Linz,
Tel. +43 732 77 77 02 - 0, Fax +43 732 77 77 02 - 20. Of course, you can also contact another notary of your choice.

Please note that the notarial act is NEUTRAL, i.e. not exclusively for the implementation of treatment at the Kinderwunsch Institut Dr Loimer. Furthermore, this must be created in accordance with Austrian law.

Important: If treatment with egg or sperm donation is planned, an additional notarial act is required for married couples. In the case of unmarried couples, the general notarial act can be supplemented with the passage for donation.
